QCTO Form 5: Programme Delivery Readiness Evaluation Tool
QCTO Form 5 is used in the accreditation site-visit/readiness process to evaluate whether a provider is ready to implement the programme. The current QCTO provider guidance identifies Form 5 as the Evaluation Tool for Programme Delivery Readiness.
What is QCTO Form 5?
Review your evidence against the actual criteria before the visit. Organise the documents, people, resources, learning material and operational evidence that demonstrate readiness rather than treating Form 5 as a paperwork exercise.
How to use this guide
Start with the registered qualification or programme you intend to offer. Build an evidence matrix, assign responsibility for each item, check document validity and version control, and make sure the evidence can be demonstrated in practice. A strong application is not simply a collection of PDFs; it is a coherent operating system for delivering the programme.
Official QCTO reference
QCTO states that Skills Development Providers offering programmes and qualifications within the OQSF must seek QCTO accreditation and comply with the applicable minimum criteria. The official provider guidance also provides access to accreditation forms, criteria and programme information.
